Commercial Award: The nominee must be a business which goes beyond the requirements to remove or alleviate either architectural or attitudinal barriers to benefit the disabled population of the community.

            Required for submission:

          • One Letter or article which shows a specific example of the business owner or lessor’s efforts

            to remove architectural an/or attitudinal barriers.


Distinguished Service Award: The nominee must be an individual who serves in a service profession who strives to reach a standard above the expected norm in their attempts to assist disabled community members in their quest to reach their full potential.

            Required for submission:

          • One Letter of recommendation

          • May be submitted (not required): Newspaper or other articles highlighting

            any accomplishments or achievements.


Employer Award: The nominee must be an employer that hires a person(s) with disabilities but in addition takes noticeable steps to integrate the disabled employee(s) into the workforce and encourages the disabled employee(s) to reach his/her full potential.

             Required for submission:

           • One Letter of recommendation

           • May be submitted (not required): Newspaper or other articles supporting the nominees efforts.


Family Award: The nominee(s) must be a family with at least one member who is disabled (parent or child). This family would be one that continually assists and encourages the disabled family member in reaching his/her full potential. The family is able to be “mainstreamed” into the community with no undue emphasis on the disability.

              Required for submission:

            • One Letter of recommendation

            • May be submitted (not required): Newspaper or other articles highlighting

              any accomplishments or achievements.

Mayor’s Award: Nominee must be a person with a disability. The award is based on accomplishments and/or achievements since the onset of the disability.

              Required for submission:

            • Informal resume including education, employment, personal or civic community accomplishments                and/or achievements.

            • One letter of recommendation

            • May be submitted (not required): Newspaper or other articles highlighting

               accomplishments or achievements.


Media Award: The nominee must  be an individual or group in print, radio and/or television media who has strived to provide coverage of the disability issues and has ensured that the coverage is done in a professional and sensitive manner.

              Required for submission:

            • One letter of recommendation from a person with a disability in the

              Community regarding the date, topic and manner (professional, caring,

              sensitive, etc) in which the material was presented.


Volunteer Award: The nominee is an individual or group who has made a significant contribution to the community which resulted in assistance to the disabled population that in turn assisted an individual(s) to achieve his/her full potential.

             Required for submission:

           • Statement in regard to the groups in which the individual(s) are volunteer members

           • Proof of nominees contribution such as a listing of specific actions/responsibilities.


Caregiver Award: The Caregiver Award is reserved for an individual who provides quality and compassionate care to their client. The Caregiver may be either a paid or familial individual.  

          Required for submission:

          • Statement regarding work ethics from Caregiver’s Employer

          • Listing of specific actions/responsibilities/fun outings/activities

          • Statement from Family if the individual who receives care is a minor
